ONLINE ENTRY FORM 2019

PLEASE NOTE:  This form cannot be submitted directly.  Instead, please email it to us by carrying out the following simple steps:
   (1)  Copy the form (Ctrl-A then Ctrl-C);
   (2)  Create an email by clicking on RichardGH@nswjcl.org.au
          (make sure the email is in HTML format, not plain text);
   (3)  Paste the form into the body of the email (Ctrl-V)
          (if the form is no longer in tabular format or there are no colours your email is probably
          in plain text format – go back to step (2) above);

           Primary Schools Coordinator (9498 2760) to determine grades.

1 All schools that have fielded teams in previous years should consider
   entering at least their strongest team in Championship Grade.

Intermediate

   your zone, then these teams will be allocated to:
   (indicate the number of teams in each).
